I've had this problem before. Two things to check: (1) the DLL's may still be locked in memory if you just shutdown Oracle; try rebooting with all the Oracle services disabled so that Oracle doesn't start up at all then run the patch or (2) are the DLL's marked read-only? I know it sounds strange, but I've seen installation programs mark all their DLL's read-only "for protection."
