In order to upgrade Menu5.0 to Forms4.5, you need the Forms4.0 tables. Somewhere in your ORACLE_HOME directory (together with the F45BILD scripts) is a F4BUILD (or something like that) script. This script contains the Forms4.0 CREATE TABLE statements. Run this one as SYSTEM, and the upgrade should work (I hope).
