Convert SQL*Menu 5.0 to Forms 4.5

I'm trying to convert a SQL*Menu 5.0 menu app to Dev/2000 (Forms 4.5) Rel 1.2 menu. I have tried all of the generate options I can think of but I always get the message:

FRM-10043: Cannot Open FIle

I am giving the generate program the following parameters: Menu Name, DB login/pwd, type= menu, location is database, upgrade=yes

I need to get this converted soon.

Any ideas would be appreciated.
