I agree with you, most down time is due to human error, or applying something (like an upgrade) to existing systems.
I don't understand how management decides whether or not to spend money on training or on shoring up procedures to reduce down time, I suspect they only do it when there is a "perceived need" - i.e. when something bad has happened.

Given that "most" db problems are app-centric, and a healthy majority of *any* datacenters' downtime is human-error related, it seems to be that the route to stability is through the enhanced learning of the operator/developer/administrators.
