I found this in Forms 6 online help in 'About implementing menu security'.



Note: The FRM50_ENABLED_ROLES view must exist in the database for menu security to function properly. FRM50_ENABLED_ROLES is automatically created as part of the Form Builder 5.0 table scripts that run during installation if you choose the "Create database tables" option. If you did not choose that option, you or your DBA must logon as SYSTEM and run the SQL script FRM50SEC.SQL (located in the Form Builder SQL directory).

This script must be present on the CD from which you installed Form Builder. Does this help?
