In my experience if ASM is saying the disk doesn't exist it's likely for good reason. There could be a few things that were missed along the way.

What OS? How are you mapping device to your /dev path (ASMlib, UDEV, etc...) ? Does the /dev path actually exist? What are the disk's permissions? What OS account is ASM running as? If you're using ASMlib, What does your ASM disk discovery path currently look like? Has the disk been 'stamped' with oracleasm prior to your attempt to add the disk?
