Can't really speak to the optimizer, but there are a whole slew of bug fixes between 11.2.0.3 and 11.2.0.4, especially in the realm of RMAN and standby databases. From that perspective, making full use of physical standby databases is night and day between the two maintenance releases, as the bugs afflicting 11.2.0.3 are about as painful as it gets, forcing one to take the standby out of managed recovery mode just to get an incremental backup. In 11.2.0.4, the bugs all disappear and everything works as it is supposed to.
