In my mainframe days...
Brand-new custom-built manufacturing floor control system, just completed conversion that morning, starts slowing down and grinding to a halt by 1pm. Outage lasted about 16 hours. PR disaster for my project.
Cause: Contractors doing some kind of cleanup in the floor above the data center left a water hose running while they went to lunch. Water soon dripped through the ceiling into a 3088 switching unit, causing more and more paths to short out and force re-tries on other channels.
Very fortunately, there was a unit across the street that could be moved in to replace it.

All systems and storage and everything electronic shut down completely.
Data center located below ground level and a major water main break just outside the building.
