No, you don't have to do a custom install - but yes, you do have to deselect them in the EM Setup (I already provided instructions on how to do that)

Database Control is only for a single instance anyway. Grid Control is the product to use for multiple instances, but yes, you're right - you have to pay per instance for the management packs.
