my favorite book is still "Relational Batabase Writings, 1989 - 1991"
by C.J. Date and Hugh Darwen, ISBN 0-201-54303-6.
check out chapters 13, 17, 18, and 21.
Then there is the famous "Database Systems" (8th edition) by Chris Date, but it is a rather thick book with only one chapter about missing information.
