Usually this indicates a coding problem, you may not see it on other
databases, because someone hasn't yet encountered the error situation. You
might want to gather more information about the piece of code where this
error happened and let the development team know.
Have you altered any column sizes lately? That could cause something like this because some developers do not prefer anchored declaration (e.g. myvar table.column%type) and instead prefer (myvar varchar2(20)) instead. If the column size is not changed to 30, but the variable declaration is not changed, you might errors like these.
