They may be many reasons for this.
If you have an access to Metalink, there are several notes on this = there.

If it used to work, you should first check for a filled file system : = if
Oracle cannot create a file in $ORACLE_HOME/rdbms/audit, connect = internal
will fail.

The password file won't help : it's only useful for login as = non-trusted
user.
