I know this isn't what you asked for, but it might help you find the reference material that you looking for.

I recently found a book titled, "ORACLE Developer/2000 Handbook" by Michael Stowe (Prentice Hall Publishing). I believe that he has put out a book on Designer 2000. The Dev/2000 book is fair. Good examples, but lacking in advanced topics.
