Andy,

That means Oracle cannot find any way to resolve the actual location of the instance to which you are trying to connect. It sounds like someone has changed something like the client side tnsnames.ora file which contains the actual information required for Oracle to find the instance, or something else depedning on how client connection requests are resolved.
