Actually, yes. A long, long time in a galaxy far, far away I was working for an HMO and had the same problem. The solution was found in a brilliant peace of software called "Move4Servers" and produced by Princeton Softech. This software enabled me to extract logically consistent subsets of the One Huge Database (the main database of the company, which used to be known as "PULSE") and give them developers to develop. The baby databases contained all the structure needed by a particular development group. It worked great and I can really recommend the company.
