did you set the environment variable ORACLE_HOME prior to applying the
patch?
did you issue something along the lines of
perl <path_to_OPatch>/opatch.pl lsinventory
prior to
perl <path_to_OPatch>/opatch.pl apply
If it does not see the rdbms components, something must be seriously wrong.
