Creating Second Database

Hello!
I've installed Oracle 8i on a Windows NT 4.0 box and was able to get one database / instance up and running. However, when I try to create another database and instance using Oracle Database Configuration Assistant, I get error #ORA-12560, "TNS: Protocol Adapter Error." This error seems to occur after the second instance is created and started. I realize I'm not providing a whole lot of background info here, but can anyone tell me where I need to start looking in order to correct this problem and get my second database built?
