UTL_FILE package version mismatch

When ever I try to use the UTL_FILE package I get a message telling me that the package version does not match that of the server. I forget the actual error message but I think it is something like ICD vector error.

Has anyone come across this problem and if so, what was the solution?

Any help would be greatly appreciated.
