Have you check the directory permissions, and the disk space avaliblity for the location of the orapw file. You also want to make sure that the _<SID>sga.def file is not being placed on the disk unit - it could be taking up all the disk space. I would have the SA monitor the disk usage while you create the database.
This is just a suggestion without knowing your database and/or system.
