You asked a number of questions in your posting (regarding a non-trivial task), of which I'll just address a few.
When you refer to "My users will have SLIP/PPP accounts with ISP's around the US. They'll dial in to the ISP", I assume you're referring to their own ISP, and not yours. If you already know who your customers are, then let them dial directly into your machine, and bypass the ISP altogether. I imagine this is not what you're seeking, though.
As well, keep in mind that if a client was inside a firewall, their firewall may not permit the network application protocol "SQL*Net". Many firewall vendors are or will be providing support for SQL*Net through their firewall, but if a client doesn't have this, they may have difficulty getting to your database.
Another thing to keep in mind (my marketeer hat is still off), is that Oracle will be releasing very shortly a version of Developer/2000 which will take an existing Forms application and create the same type of application for the Web (I have seen this, this is not vaporware). Pretty cool stuff.
