What version OMS, O/S?

On NT, one may check the client's event log for application-related incidents. You may likely see an event to the effect that OMS failed to start, check (your Oracle_home)\sysman\log\oms.log for details. That log, or one similar, should contain the ORA error and possible explanation. For example, my problem has been a changed username or password for the OMS repository. I use the configuration assistant to update the password, and ensure it's changed in the profile dirtree.

Do you have access to Metalink? There are a number of good hints to be mined there, IMHO.
