You can not convert forms 3.0 to 4.5 using CASE Designer If you have Designer/2000 tool,
you can do it by Forms generator component with Upgrade = Y option

Once it is converted to 4.5 version,
it can be reverse engineered in to CASE Designer tool But the converted form will not have the GUI look

There are some third party tools which will convert Forms 3.0 to 4.5 with GUI look (one I know is at www.kumaran.com )
