To add a little here. Yes 11.2.0.4 is it's own install just like Chris pointed out just make sure you delete the Oracle_Home environment variable before you start the software install, you can put it back after the software is installed. Regarding PSU's, they have been converted into Bundle Patches (BP) for Windows, I believe you don't have a choice. This includes security patches and but fixes. I would make sure you get the latest one as there was some issues last year with memory leaks and breaking database links in a few of them. If you db links to other 11g databases make sure they are upgraded too or you might have issues. Do your own tests.
As for the actual upgrade, just run Database Upgrade Assistant (DBUA) and it will walk you through the upgrade. Warning though for me I had numerous errors during the upgrade process that I had to click "Ignore" on for the upgrade to finish and dealt with them afterwards. Be prepared to open and SR and deal with any components not valid after the upgrade. You'll need to test this many times.
