Nothing might be wrong with this example. Basically, the collision is when ASM striping undoes hardware striping to a certain extent.
It may happen if you are unlucky enough to have certain combination of:
- Number of disks in the striped set - Number of LUNs from that pool - Stripe sizes - Extent locations
