This is not new.

I've encountered this as far back as 7.0.x.

No root cause is ever found as we have never been able to reproduce this. Normally it only happens very infrequently.

The solution has always been:

  1. kill -9 PID
  2. kill the session
  3. if that doesn't work, shutdown abort and restart.
