Doku's great (http://asktoad.com for example), but remember to force an SSL connection from the webserver if you plan on storing sensitive data like passwords. There are Apache MOD_REWRITE examples of how to do this that Google can find.

There are quite a number of tools, many free/open source, that would work for this purpose (a DBA intranet site). You coud use a Content Management System or, possibly better for this purpose, a Wiki such as DokuWiki.
