Are your two standbys on the same server? Typically they would not be.

Each standby is configured independently from the other -- however you set up the first one, the second should be handled essentially the same way. So the answers to questions 1 and 2 are "yes".

Your standby controlfile should be created after you create the hot backup, as per the documentation.
