We're happy with VSS. If you happen to have SQL Navigator you can buy a feature which will export the current version of all your code to VSS which saves a *lot* of time with initial setup.

I don't know if you can really use it for data models though, at least we've never tried to.

We keep each database as a project, with Production, QA, and Development as sub-folders, then each schema as sub-folders under those.

In a few cases we also have a subfolder for unix batch scripts on the same level as the schemas.
