Query Medical Claim

Hi,

I need some help here. I wanted to generate a report showing : emp_name, emp_id, med_id, medical_type, entitlement, used, balance.

TBL_EMPMEDICAL
emp_code, 
emp_id, 
med_id,

amountused  

TBL_MEDICAL_TYPES

Med_ID  MediType      Entitlement


------------------------------------          

M01     supmed        unlimited
M02     execmed           5000
M0      execmedopt        8000  

Each type of medical type have max amount of entitlement

I really need to solved this quick. Can someone help me out with this.
