I'd be logging a Tar as well at this point as a brief search on metalink suggests a number of potential causes for the 12500 as well as those already mentioned.

Oracle Trace enabled.
Oversizing the SGA and running out of process memory for Oracle.exe
misset ORACLE_HOME
no permissions on Oracle home for the account that runs the service (normally this would be the local system account on NT.

etc etc .
