I do like to ask the questions:
"What has been the biggest DBA work related mess-up you've done?"
"Why did it happen?"
"How did you fix it?"
"What did you learn from it?"
"What have you done to avoid this in the future (and to help others not to
do this kind of thing?"

The same goes with:
"What has been the biggest challenge in your DBA-work?"
"How did you solve it?"

Also, I like to ask a lot of "why" questions, this helps to select out people who understand the fundamentals instead of people who have memorized half of the Oracle reference guide.
