The problem is that the patch may cause an unexpected side-effect in another part of the code, or when some feature is used, but wasn't thoroughly tested for. If you read Oracle bug reports, you will see many examples of these types of side-effects. Even with the comprehensive testing performed on a complete Oracle version or patchset, bugs are still overlooked, hence the reason for bug reports.
With a one-off patch there is typically no opportunity for the software vendor to marshal the testing resources needed to thoroughly test the now-new version of the application. To perform that degree of testing would require delay in the availability of the critical patch. That is why a prudent person only applies patch sets because these have some degree of comprehensive testing. Even better, wait until other sites have had the opportunity shake out the bugs from a new patchset before applying it to your most critical applications.

what problems did you have with one off patches? This is a critical error
that can interrupt service.
