Why do you have to have php?
Htmldb would do the trick in all likelihood. It comes with 10g and can be back ported to 9i.

Subject: HTML forms generator which can generate basic forms with Oracle and
PHP
I am about to start writing an application which should have several HTML
forms
which should be used to access the database. Does anybody know of anything
suitable,
preferably shipped under the GPL license?
