In a generic way, I can tell you why.
It's Windoze. Win2k is better at starting services at bootup than Windows 4.0 was, but it still is not completely reliable.
Which is why I have a service to monitor services on other boxes, and restart them if necessary.
Yet another server has a service that monitors *that* service.
Funny, never have to do that on *nix.
