You can read BFILEs without having client access to the file. As long Oracle has access to the file, it can open it & stream it to you. But if you want write access to these files you have to do it from OS or with utl_file or similar packages. Gets too complicated to use if your end users want to modify those docs, but it could work if you're loading your doc's once and they are only read afterwards.
But yes, internal lobs can be fast if implemented properly.
