Paul,
Make sure you're on the lastest 8.1.7.4 patch level.
Learn about 10046 extended SQL tracing. Then, arrange for one or two of those users who have been complaining to call you as soon as they notice something fishy and turn on the tracing. This will show you in more detail than you might care about what is causing the wait.
TOAD and Spotlight are useless in these circumstances.
