Jared is probably right as far as a checklist for the actual upgrade. A test run through will probably highlight any problem areas.
After the upgrade there are a number of things that usually need to be checked/modified/recompiled (of course it depends on how you upgrade). Here is a list that I found useful. Please add to it if you wish.

The best to do list will be the one you create yourself.
You can do this by reviewing the migration manual, and determining which steps are appropriate for your environment.

I am looking at a project to upgrade a number of Oracle development, test and production DB's to newer releases. I have reviewed Oracle's list of tasks from OTN. I would like your feedback on other tasks, etc. that should be included. Is there a list somewhere on the web that gives a comprehensive list of tasks that need to be accomplished and "gotchas"?
