In TOAD 7.5 (beta), you can choose ER Diagram from the Tools menu. You might have to use the menu editor to add ER Diagram to the menu; I don't recall. Or just right-click a table name in the schema browser Tables tab and choose ER Diagram from there.

I'm not sure how much of the ER Diagram tool is in the commercial 7.4 version of TOAD. I know they've enhanced it a lot for 7.5.

It's not a full-blown PowerDesigner clone and probably never will be. But for simple diagramming and such, it works great.

Any registered owner can download the latest TOAD 7.5 beta at www.toadsoft.com/beta.zip. A new beta is realeased every Monday and Thursday.

Just copy your current TOAD directory and install the 7.5 beta there on top of the copied 7.4.
