We change production password too, but not that often. But I have a DB level trigger that logs each login and captures OSuser too. Then I have a report that is run by my good friend "cron" telling me who is logging in as what. So, if we find somebody, their managers get informed, mentioned in the weekly meetings and schema password is changed.

Now, our developers know the big brother is watching and yes we do capture any and all DDLs that run on the system (along with osuser), so we can go back and see who did what. This is not intended for policing, but mainly to see if someone did something without telling us.
