This is an ugly answer but here goes.
Shutdown all of your instances, disable db_start, bounce the box, manually start each instance after you are absolutely sure about where $ORACLE_HOME/dbs/instance_link is pointed, start each instance, run catalog and catproc.
The 8.1.6.3 patch is a very ugly puppy. Read the instruction very, very closely!
