It's usually accessed from a Java program - which could be a Java Stored Procedure. You can download it from this URL:
http://technet.oracle.com/tech/xml/oracle_xsu/
There's documentation and sample code there too. In addition, it can be accessed from PL/SQL, using the supplied XMLGEN package.
