The first big whoops in my career was a co-op assignment that I did in college. I analyzed and designed an entire system, with a couple dozen programs, only to have it ultimately be a complete flop. It didn't do what it needed to do, everything had to be trashed, and a new design effort had to be started. They even brought in a new "lead" to replace me, but they kept me on the design team. It was a fairly high profile project, and I was absolutely and completely crushed. I felt like a worthless SOB who could never amount to anything after that. I'll always remember though, how gracious my manager was in handling the situation. There was no blame. It was just "we screwed up and we need to start over".=20

I guess that's a different type of "whoops" than deleting the wrong database tables, but I've made my share of both types.
