As Forms 4.5 is an old version, I believe it is not certified with MS Windows Vista and - in my opinion - never will be.

In order to find out whether an Oracle product is certified with a certain operating system, check the Oracle's certification matrices.

Do you want to run Forms 4.5 directly on Vista or display an application which is running on a Linux or AIX on the screen of a PC which is running Vista via an emulator?
