How to runtime without the default Tool Bar?

for example I found this file ( the attached file ) on the some forum , that file run without default tool bar.... that's the default Tool Bar :-

How can remove it and create my tool bar?
or at least how can I make new form like the attached file?

note that when I copied the properties like (triggers , blocks ....) from WINDYNM to MODULE3 (MODULE3 that is the new one that I made) give me the default tool bar with not wanted result.

Forms' "Menu Module" property is, by default, 'DEFAULT&SMARTBAR". Remove '&SMARTBAR'.

I thought I wrote it simple enough ... obviously, I didn't.

'Menu Module' property should be 'DEFAULT' instead of 'DEFAULT&SMARTBAR'.
