Search this forum for 'convert', 'conversion', and 'migrat'. Basically, use Forms 4.5 to get the forms from 3.4 to 4.5 and then use Forms 6i to get the forms from 4.5 to 6i.
There are products 'out there' that say they will do the conversion for you. Google it.

IAG (or Interactive Application Generator) is a very old SQL*Forms utility used to convert FORMS from INP to FRM format. If you run it interactively, it would prompt your for table names, column names, screen positions, etc. and generate the FRM from the input provided.
However, that is all ancient history now...
