It is not clear to me what you are asking.

If you want to know the name of a form in Oracle Applications, then navigate to the form and do Help, About Oracle Applications, then scroll down (about 3/4 of the way) until you find a section called "Current Form". This should provide the information you require.

If this is not the question you are asking, can you please make your request a little clearer?
