I have created a form with database and also i have entered data.
but how i desgin a report according to attached file.

I am trying but how create ??? in matrix or matrix with group etc....???

please help me

I don't think u need to prepare a Matrix or Matrix with group...
I think its more of a simple group above report..
bcoz a Matrix report should atleast have four sets of data as defined below:

A cross-tabulation of four sets of data: one set of data displays across the page; one set of data displays down the page; one set of data is the cross-product, determining all possible locations in which the across and down data relate and placing a cell in those locations; and one set of data displays as the "filler" of the cells..

But ur Report o/p doesn't look like..
