Im extremely FURIOUS so ****-off if you are going to be rude after reading this.
Ive been assigned a task to modify form in Forms Builder.
So I did what I was asked for, compiled form, deploy, created DPPL record in Function and so on. Launched it from EBS, it starts, but ::
1. There are two empty panels on both sides, it looks hacked. See blocks.jpg
2. My form is in file XXPOXRQERQ.fmb, it opens, lets me edit it, when I want to save it, there is another name proposed. Whats strange when I hit ENTER, its saved OK under the XX preceded name.

BLOCKS.JPG - I'm sorry, but - which two empty panels? Could you mark them (using Paint, for example)?
Form name (the one you see in Property Palette window) can differ from the .FMB file name (which happens, I presume, in your case). It would be simpler, easier to maintain and more obvious if those two names are equal (at least, that's what I do all the time).
