How about some more information? For instance:
What was the output of the CVU ? What release of Grid Infrastructure (do not say "11g". Say, for example, "11.2.0.4")? Is your software 32bit or 64 bit? What about your Windows? What is your network configuration? What is the storage?

I would not even try to fix an 11.2.0.1 installation. There were significant changes between 11.2.0.1 and 11.2.0.2. If you have a support contract, you should use 11.2.0.4. If you do not have a support contract (ie, this is just for your own learning) then use 12.1.0.2.
Your network looks OK, but you have said nothing about storage. Is it shared? If so, how?

Prove it. You may find that
runcluvfy comp ssa
is useful, as well as several other cluvfy comp commands to check your environment.
