What criteria is used to determine whether an application requires a new Instance be created versus
just creating schemas in an existing Instance?

For the same application I recommand you, for performance and administration reasons to use only one instance and as many schemas as you want.
If the new instance wouldn't have any data related to the data of the existing instance and the performance of your existing database is low it maybe better to create a new database on a different server.
